Why people leave Epic

Epic is the electronic health record that large hospital groups choose when they want one system for the whole organisation: clinical notes, orders, medication, scheduling, billing and a patient portal.

Its own site says it began in a basement in 1979, now has more than 325 million patients with a current record, and calls itself a software factory with no acquisitions. It also claims the top spot in the KLAS overall software suite ranking for sixteen years running, which is Epic's own statement.

Epic Systems Corporation is based in Verona, Wisconsin, and calls itself employee-owned. What we could not find is a public customer agreement or a price list: its legal pages hold trademark and usage material, not contract terms, so what law applies to a European hospital and which entity signs are matters you settle in a private negotiation. This page does not claim to know them.

The tools below are European electronic patient record vendors with a visible head office, mostly with a public register entry. Eleven qualify, and they are not interchangeable. Some run whole hospitals, one is an openEHR platform you build on, and the last few are primarily GP and practice systems, which only replace part of what Epic does.

  • No public terms or prices to compare Epic publishes no customer agreement and no pricing that we could read, so a European hospital cannot check the contracting entity or governing law before the sales process begins. The European vendors here mostly state a company and a register entry on their own sites, and several name their group or owners. None of them publishes prices either, so this is about knowing who signs and where they sit, not about a cheaper list price.
  • One system for everything is also one supplier for everything Epic's strength is a single integrated record across a whole organisation, and it says it builds everything itself rather than buying other companies. The trade-off is dependence on one supplier for clinical, financial and patient-facing tools. A European buyer that wants a different balance can look at a hospital system from one vendor and specialist modules from others, or at an open-standard platform such as Better.
  • European vendors already run national and regional systems ChipSoft says many Dutch hospitals use HiX, and UZ Gent and AZ Ganda in Belgium chose it. Cambio has served Swedish hospitals since 1998, DIPS serves Norwegian hospitals, and TPP says SystmOne is used by more than 2,600 GP practices in the UK. These are the vendors' own statements and we did not audit them, but they show the market is not empty.

European ownership of the vendor does not remove every dependency, but it is checkable, which a hospital buying a record for fifteen years should want.

What you have to replace, not just match

Start with what Epic does for you today: inpatient records, outpatient scheduling, medication management, revenue cycle, a patient portal and reporting. No European vendor here covers all of it in one product in every market. Cambio, ChipSoft, Systematic, Dedalus and DIPS come closest to hospital-wide systems; Better provides the data platform to build the rest on.

Ask for a reference you can visit, in your own country, at your own size, and for the contract terms before the demo: contracting entity, governing law, data location, and what happens to your data if you leave. Plan a phased move by department. Replacing a hospital record is a multi-year programme, and no vendor on this page makes it a quick switch.

Its HiX record is described as a fully integrated solution for hospitals and clinics, general practice, rehabilitation, mental health and other care settings. A customer on its site says that a large part of Dutch hospitals work with HiX, and the news section says UZ Gent and AZ Ganda in Belgium have chosen it as their new patient record.

No prices are published. Its documentation and site are mostly in Dutch, with an English option.

Cambio was founded in Linköping in 1993. Its history page says it won its first hospital customer, Ersta Sjukhus, in 1998 and its first county customer in Huddinge, and has over a thousand employees. Its privacy policy names Sanolium Group Holding AB as the parent of Cambio Healthcare Systems AB, Cambio Healthcare Systems A/S and others.

The offer includes the COSMIC record, VIVA, an anaesthesia and intensive care record, decision support and an openEHR product. It has a Danish version of its site as well as the Swedish one.

We could not find a public organisation number, and the site is mostly Swedish. No prices are published.

Its healthcare pages describe Columna CIS, an electronic medical record for hospitals, with modules for medicine, booking, requisitions and patient administration, plus Columna Cura for community care and Columna Flow for task management. The company also builds software for defence, police and libraries, which is a different profile from a pure health vendor.

It says it has a strategic investment from Ardian, which describes itself as Europe's premier private investment company. The company grew through an expansion strategy that began in 2016.

DIPS AS is a Bodø-based software house with about 400 employees that develops healthcare technology for Norwegian hospitals, with more than 100,000 users in the health service. Its postal address is Postboks 1435, 8037 Bodø. It says it has been developing since 1987 and is part of the Kernel group.

Its products include the Arena patient record, a mobile app, DIPS Assistent with AI, and tools for patient involvement and connectivity.

The site is in Norwegian, and its focus is Norway. No prices are published.

The Phoenix Partnership (Leeds) Ltd. is registered in England and Wales (number 04077829) with its office at TPP House, 129 Low Lane, Horsforth, LS18 5PX. Its About page says it was founded in 1997 by Frank Hester and that SystmOne is used in more than 25 care settings, including more than 2,600 GP practices and a third of acute mental health trusts.

Its hospital modules include bed management, PAS/HIS, emergency department, e-observations and order communications. It also has deployments outside the UK, in China, the Middle East and the Caribbean.

What actually breaks when you switch

Replacing a hospital record is a multi-year programme, not a licence swap. Plan a phased move department by department, keep the old system readable for the legal retention period, and budget for training and for rebuilding order sets and workflows.

None of these vendors publishes prices, so you will compare quotes, not price lists. Ask for total cost over the life of the contract, including hosting, interfaces and change requests, and ask where the data will be kept.

Local fit matters more than features. Most of these vendors are strongest in their home country, language and reimbursement system, so check references in your own market before you shortlist.

Which of these can replace Epic in a hospital?

The best candidates for a hospital-wide record are ChipSoft HiX, Cambio COSMIC, Systematic Columna, Dedalus and DIPS Arena. Each describes a system for hospitals on its own site, and each has deployments in its home country or region.

Match the vendor to your country and language first. Most have strong local roots and thinner presence elsewhere, and a hospital outside the vendor's home market should ask for a reference of its own size.

What about open standards and avoiding lock-in?

Better is built around openEHR and FHIR and sells Better Studio for building your own clinical applications on top, so the data model is open rather than held by one product. Cambio also lists openEHR on its site. An open data model helps with moving between systems later, but it does not remove the effort of rebuilding workflows.

Corilus (Belgium), Cegedim (France) and medatixx (Germany) describe GP, specialist and practice software. TPP SystmOne spans from general practice to community, mental health and hospital modules. CGM lists both ambulatory and hospital information systems.

If your hospital needs a practice-level system for an outpatient clinic, these are the ones to look at. They do not replace Epic across an organisation.

ChipSoft in Amsterdam, Cambio in Sweden, Systematic in Aarhus, Dedalus in Italy, DIPS in Norway, Better in Ljubljana, TPP in Leeds, CGM in Koblenz, Corilus in Ghent, Cegedim in Boulogne-Billancourt and medatixx in Eltville. UK-based TPP is outside the EU but is in a European country, and the register entry shows an English company.

Which one to pick

If you want a hospital-wide record in a European vendor's home market, ChipSoft HiX, Cambio COSMIC, Systematic Columna and DIPS Arena are the first four to look at, and Dedalus is the one with the widest national reach. If you want an open data platform and your own applications, Better is the odd one out in a useful way.

TPP SystmOne is the one for UK community and primary care, and CGM, Corilus, Cegedim and medatixx fit ambulatory and practice care better than whole hospitals. They are alternatives for part of what Epic does, not for all of it.

Be realistic. Epic is the record that integrates everything for a whole hospital group, and none of these eleven has been shown here to match that breadth in every market. Choose a European vendor if you want a visible company, a register entry and a reference in your own country, and ask for the contract terms before you commit.

Choose by country, size and the references you can visit.

No. Epic Systems Corporation is at 1979 Milky Way, Verona, Wisconsin, USA. Its website calls it employee-owned and says it was founded in 1979.

Epic does not publish prices that we could find, and none of the European vendors on this page publishes prices either. Hospital records are quoted per project, so expect a tender process rather than a price list.

Not that we could find. Its legal disclosures page lists trademarks, usage guidelines and a few regulatory statements, but no customer agreement. Ask for the contract early and read the data protection, data location and exit clauses before you shortlist.

Yes. ChipSoft B.V. is at Crystal Tower, Orlyplein 10 in Amsterdam according to its privacy statement, and its site names offices in Amsterdam, Heerenveen, Hoogeveen and Antwerp. We did not retrieve its KvK number.

Dedalus says it has a strategic investment from Ardian, a European investment firm. Cambio's privacy policy names Sanolium Group Holding AB as the parent of Cambio Healthcare Systems AB and its sister companies. DIPS says it is part of the Kernel group. We did not trace deeper ownership chains for any of them.

None of the eleven is open source in the sense that you can download and run the full product. Better builds on the openEHR and FHIR standards, which are open specifications, and Cambio lists an openEHR product too.

The Phoenix Partnership (Leeds) Ltd. is registered in England and Wales (04077829), which is outside the EU but in Europe. It is included because this site counts the UK as part of Europe, and it is the only UK vendor on this page.

Data migration is a project in its own right. Ask each vendor for its migration approach, the standards it supports (HL7 FHIR, openEHR or others) and examples of previous migrations from other record systems. We have not verified any vendor's migration claims for this page.
